Staff Facilities Request Form Facility Use Guidelines 1. This form is intended for LMPC staff members. 2. Facility Request Forms (FRFs) should be submitted as early as possible, allowing at least 3 weeks for JHYB requests and 2 weeks for other locations. Having as many details as possible on your first submission will speed up the approval process. 3. When your event is approved, you will be notified by email. 4. Please understand that events held after business hours, even in your office, should be noted on the master calendar. We want to ensure that the temperature control is set, that the facility cleaning schedule will not interfere with your meeting, that we know of your presence should there be an emergency, etc. 5. Please remember to obtain an approved request for any event you are planning before you announce the event to the public. 6.
